Hoho! Merry Chritsmas!

* been times. yeah.. didnt updated my blog. At times, i forgot that i own one -_- Anyway, MERRY CHRISTMAS EVERYONE!!!! Let's be merry no matter how you celebrate it - whether it's big one or just family - Merry Christmas!!

My sister ordered me the Eclipse & Breaking Dawn [Twilight Saga III & IV] books for chrismas! Yes, we fought alot - but geeez, i started to think of treating her more nicely after this. Dudududu i know im the last person to have these books - but who cares ya. As long i own them. dudududu :P

and my dear bought me a Sony Music Player which ive been wanting for years. ahaha yes, years. Poor little girl that i cannot afford it myself. When he said ' the thing is orange in colour '.. I expected it to be toys or dress or any orange can represent for. hehe but Im soooo glad it's the MP :D

We have a little christmas party at home - it's emm.. nice but something really bad happen in the midnight.. i seriously dont think alcohol is the perfect drink during christmas.

Twilight n im in love.

Went out wt my sister dis evening for movies. been craving these for a week already! we have 2 movies in row - TWILIGHT & 2012. i love Twilight. both seasons. i love all d words Edward said to Bella! he’s sooo charming!! it’s funny how me and my sister changed our thought, i wanna see 2012 but i didnt actually really like it in d end and my sister choose to see Twilight at d beginning and ended of disliking it. haha I cant wait for d next chapter of Twilight.

i went to Popular for Twilight Books but it’s so devastated. How come it wasnt there when i need it? yes, after TIMES. lol

RMWD Oct 2009

Hello Sunday :)
1 hour to go, time flies so damn fast huh? it's going to Christmas soon! and we're thinking of celebrating it at Kota Marudu!! haha it's always nice to your loved ones around ya. but anyway, been quite busy with my new works recently, i miss blogging! for real :D

looking foward for another assignment with these lads pretty soon ^^ they're awesome! Oh, i met my cousin, Cado on the 2nd day, been AGES!! and he recognized. hehehe that means, i changed nothing and am still the same :D

been a fan with indon's tracks nowadays haha! my dearest downloaded some for me, and he's kinda into it also hahaha i think it's funny when you know he's totally not that type :P oh, and i have my shoes repaired!

Karnival RMWD Etnik Desa


Phew! Penat! baru balik from Kg. Bayangan, Keningau.
Depart from the town around 10 something, singgah Gunung Emas for pictures+ session hehe arrived safely at home 15 minutes before 1pm! It was really2 fun to be there for 3 days! Ive met our youth & sports minister (altho im not that excited :P), Ebi af, C ampal urang kita ( for few times suda) and new people, new boss, new friends - i just love it!!

Starting all over again.

I should be working on something now but i end up with FB-ing and blogging :P

i still cant imagine how my current Boss will react tomorrow! sigh. i cant find a word to say. they said i should just leave, but i feel guilty somehow. she must have done something right that makes me feel this way. is she, really? i dunno. Blank! at this moment, i am more like Que Sara Sara, whatever happen - happen. What sure, i will be going to the office tomorrow for the final works, handover and packing my stuff out.

I hope i can go through these successfully:

Mixing Up like a cake.

la la la la..
okay, im totally ran out of idea to blog already. I love blogging but i'm not good in expressing words, a good execuse of why i rather love talking! cause it doesnt require me to fill the blanks to people read it through, memorize each words but still, dont get the idea. haha And good thing about talking is that sometimes they will simply forgot of what you had been said earlier altho they did have a laugh on it. nah, bingung suda sa ni. haha ba iyala, i will go straight to the point.

Hurm. first of all, i hate my work! but well, not exactly 100% hatred but at times, i get too annoyed by its surroundings. They're so freaking mean. I mean, one or two of them! A person who ask for your help but not asking in a proper way is a jerk. like, " Ninie, buat ini, buat itu!! " - tulungla, ingat sa ni officegirl? ingat kaki sa teda tol mau suruh? - hehe ba, even teda tol pun tapi shouldnt be like that la ba kan. Iyala, sa pun panas juga pigi hempas tu barang depan ko tapi sepa duluan? wakaka, mcm budak2 ni. haha but, i have to live with it. Nasib i want to gain experience. :P

Next, -_- My buddies are in Jakarta at the moment and they promised to sms me, but they didnt. Sa risau ni :P hehehe mestila, they supposedly go to Jogjakarta today for mountain climbing - yang masi aktif volcano lagi tu - and im still patiently waiting next to my phone. Siok suda kali dorang sana ni [ padahal jeles ni :P hehehe ] .. and i cancelled my trip to go visit Jkrta this coming saturday. Buli postponed ka tiket AAsia? tunggu sampai aman dulu sana. tapi sampai bila?

03rd October 2009 marked another history! One of my chilhood bestfriend get married with her dearly choosen man, Alex Wan, a very humble nice guy we've ever met. We are indeed, very happy that she found her happiness already ^^
and we were there!

Should do this again sometimes.

Hello guys, Selamat Hari raya! im back from raya. You know what im thinking at this moment? i personally think that The government should gives Malaysia more holiday rather than just two days! I had to starts working on the 3rd day of raya and it's so not kwel! No, im not celerating raya but im talking on behalf. hehehe

Anyways, i was out for two days in row since thursday. It was absolutely fun!

Thursday's evening - Jane, Willie, Me, my Dd, Leenda, her Bf and Les went to Upperstar for some refreshment. hehe It's cool, i didnt get to see them most of the days, so it's totally awesome to have some times together. In fact, it's been like.. 2-3 months i havent meet them. Oh, willie! i havent meet him since last year! and it's so great to have him with us =) and we went home nearly 1am.

and i spotted this on the side, i knew it's been there for quite awhile, i even loves how they decorated the walls with few guitars.

well on Friday's evening, i planned to meet my friend, Grace since she'll be off to Kuching this sunday for her Bachelor. I promised her a day out so i called Bbey, Beat, Leenda, Ivy, Siah, Jane, Am, Andy to join us but only 6 people can make it. Still fun anyhow =) We first moved to Karaoke Box and then a G-Force Movie and Upperstar. again. hehe Upperstar is a great place for hanging out. We talked about the good old memories and have some laugh of it, i cant believe it's going to be 1 year since the last day i was in the campus. Time flies sooooooooo fast! Im glad it did, but sometimes i wish.. you know, like, there's a moment/point in your life that you wish the clock will just stop. right?

Anyway,
we went home sharply at 12 midnight.

oh, for your information, i gain another 2 Kg! that makes a total of 49 Kg! no, it's not normal but i hate diet! Im fat, and im okay with that... cheh! kunun la hehe

One hour and a half journey to Kota Belud, to witness these two lovely couple ex-change rings was awesome! I met Agnes for the first time at Dd's bro's wedding's reception. She was absolutely sweet and they made a perfect couple ^^

A ceremony wont complete without ACARA MAKAN!
The foods taste superb! i love the sweet + sour + spicyless Fish.
No exact date for the wedding yet, but hopefully soonest! so i can eat more of this fish! haha

i joined my Dd and his family to Kota Belud and Kota Marudu yesterday. Phew! long journey but It was awesome to witness Raymes and Agnes engaged! Congratulations!! Will share the pictures sooner :)

anyway, me and my sister managed to catch the time to see Floyd & Amy's wedding situated at Malanggang Tamparuli, i hope i spell it right! last saturday..
They're so lovely!
George Lian entertained the guest with his latest song " Aiso Nogi " [ No more / Tiada Lagi ], and i fell in love with that song, it reminds me of my dad! Not the lyric, but George Lian himself, my dad into Dusun's song so much! and he loves singing it too. He have a great voice indeed :P hehe mesti mau puji bapa sendiri! hahaha

2 Weeks before the Wedding.

was at the bride-to-be's house last night, for the flowers, car's decoration and bridemaids' stuff. It was cool indeed, my bestfriend is getting married in about two weeks from now, such a happy feeling to me. It feels like yesterday when we were small, talking about guys and stuff like that.. we'll be walking with her in the altar, the promise will be fulfilled! hehe

Final Destination

My colleagues and i went for a movie this evening and as a record, there was only 8 of us inside the theater! Our screaming echoed the entire spaces and we decided to move from a single seat to the Couple seats haha lots of empty spaces kan.

This is the summary of the FD 1..

Alex and a group of high school students take a flight to Paris for a French class trip. Before they set off, Alex has a premonition of the plane bursting into flames minutes after take off. He tells everyone to get off the ill-fated aircraft. Moments later in the departure lounge the student see the plane explode before their very eyes. Now the FBI thinks that Alex had something to do with it and follows his every move. His friends start to believe he had something to do with it also and slowly fade out of his life. But now, each one of his friends is mysteriously being killed by death itself. Alex is now trying to find out who is next in death´s list. But, unfortunately, "You Can´t Cheat Death".

and the plot goes almost the same from FD1 to FD 4.
I have seen all of these movies so far.



we cannot cheat death if the time has finally come, but i don't wanna end my life in a tragic way. but who us to predict kan? huhu Palis2!! The FD 4 was scary tho! at certain scenes, i looked away, closed my ears and SmSing my Bf and sister. haha Anyway, it worth watching. You should go watch it too but im not recommending it to Golongan Emas or Children. Sa yang muda pun sikit mau kena heart attack! :P

Reservation by God

I told you about the interview ya. I am still actually worried about it, I had a little thought of myself, practically cause I was acting more like It's okay attitude or maybe for some people, i seems to not take it seriously. As if the chance will comes everyday with a single finger's click. I do indeed but i guess it doesnt shows enough how eager i am? But well, the mind stops working when i read Connie's comment towards it.

well dont regret k...if it has been reserved for u (by God) then it sure be urs....

God..Sure He is, Sometimes i tend to forget God's existence in every-day-life. I missed to say thanks or a little prayer in the morning. Neglection is not what i was born as, but i choose it. It is not what i am destined for, but i did it somehow. It's a pretty shameful for me. He reserves places for me, In any homes i will be. I know, i wont be stranded alone.. and still, i repay Him nothing.

Nervous? Inda juga.

09.09.09 - more than 100 couples in China get married on this day. i say, wow! if you watched the TV3 news, u can see they made a large red banner with chinese words on it and some even lined up to register. They dress casually. Mcm yang main-main kawin seja ni hehe Two of my colleagues pun MC since they have two separate wedding's receptions. siok kan.

Anyway, i went for an interview yesterday!!! honestly? I was BLANK! macam nda pernah datang interview ni. Even itu penolong pengarah noticed how blank i was, which is Embarrassing! He asked me:

" Kenapa ko ni? ko nervous ka? "

i goes, " emm iyaaa.........hehehe "

but i think the funniest among all of the questions, was when he asked me:

"Katakanlah ada menteri datang d tempat kau satu hari, apa persiapan yang patut kau buat? "

i goes, " cari maklumat, bla bla bla bla bla" [ waa.. panjang! ]

He then said, " Okay, dan yang paling penting antara semua apa? "

Confidently i answered " Makan - Makan! "

wth. makan-makan? the interviewers laughed as you can expected. And they continued asking this,

Them : " Berapa Jumlah Parlimen di Sabah? "

Me: " Okay.. Parlimen?.. setahu sa, setiap daerah ada parlimen.. Ranau satu, Kota Belud satu.."

Them: " bagi sa jumlah ya, bukan dimana "

Me: " em.. "

Them: " Tia tau? "

Me: " bukan tia tau.. tapi tiada exact number "

Them: " ??.. tia tau la tu.."

Them: " semua ada 25 parlimen.."

Me: " oo.. 25 ka semua? "

Them: .............................

ok, baiklah. i understand haha.. uhh when i refreshed back the session, im ashamed. I should have answered better. I did take it seriously but i guess it wasnt good enough? Still have times and hoping i will get a good news from them. Or a second Interview?

The man and His wife.

Happy Saturday =) times fly so fast, right? It's nearly end of the year already. Anyway, i want to share with you guys about this annoying story! Where do i starts?.. haha Okay.. Let me introduce you to the persons involved first, ya.


Sabri: Driver cum officeboy
Nina : Driver's wife.
*Samantha : Driver's officemate
*David : Samantha & My Officemate.
Me : The Driver's and & Samantha's Officemate.
Location : Wisma Merdeka, 9th Floor.

Okay, so.. how do i begin.. Well, we all aware of what driver's responsibilities right? Sabri done a great job actually, he will do the job right away once we asked him to. but the problem is, his wife, Nina cant sits calmly watching his husband doing his work! so, instead of staying at home, taking care of their children, she will wandering around EVERYDAY here in Wisma Merdeka. She call her husband every 5 minutes ( seriously! ), asking where he was AND when Sabri apart from her opr didnt answer her phone, for like, 10 minutes, she accused him for having an affair! Trust me, we can cleary hear Nina's voice through Sabri's phone even though he's 90 metres away from us.

well, Samantha used to go back home with our boss, drive by Sabri since the distance between Samantha's and our boss' house not too far.


So, One day, Nina came to the office and starts asking lots of questions to Samantha. I knew Samantha pretty well even though we've known each other only for 8 months. I am really sure she have no other relationship with Sabri but Colleague. well, Nina starts to SMS Samantha everyday, even at 3 am in the morning.. just to threaten her. No matter how Samantha tried denying the accusation, Nina wont believe.

Then, Nina started to SMS and calling our boss too!! she asked my boss " kenapa suruh2 laki aku buat kerja! " .. Nonsense right?

This situation lasted for 5 months, when it became worst last Thursday. The Boss was sick, so she was resting at home. Since she's not around, there's few urgent documents need to be signed by her. well, the boss asked Sabri to send Samantha with the documents to the boss' house. Without warning, Sabri's wife suddenly appeared right in front of the company's bus and started yelling to Samantha like a crazy people, the public gathered and looked at them. I think it's embarassing, esp when they're in the public. Nina, the sot woman, open the door and pointing her finger to Samantha - something like " awas ko! ".. My officemate, David went down to see them after Samantha sms him, and the woman ran away.

Minutes later, she called David, with so anger voice, yelling at him : sayin David purposedly ask Sabri to send Samantha so they can be together. wtf. I was mad myself too.. as i told you, the woman's voice can be heared so clearly from the distance! so, i called the woman back.

Me : hey, perempuan!

Her : Hei, ko sepa oo

Me : Oi, perempuan.. Ko ikut suka hati ko seja la kan mau marah2 orang.. ko ingat ko saturang ja
pandai mara ka, hei.. ko ingat c Sabri tu t'lampau hensem ka sampai ko tuduh2 orang begitu.

Her : aii.. kurang ajar, ko sepa. suka hati aku la. memang la sabri tu tia hensem, penagih lagi.. aku istri dia. suka hati aku la.

Me : Oi, ko jangan macam2 sana.. ko tuduh2 org seja karaja ko kan. woi, sa kasitau ko a. jangan ko pandai2 pigi marah2 c *samantha.. pigi sms dia awal pagi, ko ingat org tia mau tidur ka mcm ko. klu ko mau masi ada nasi setiap bulan, ko kasi biar seja sabri buat kerja.

Her : eeeii.. kurang ajar, siapa ni? ko ingat aku takut ko?

Me: hei perempuan, klu ko sibuk2 lagi pigi kacau kawan sa, sa cari ko. Settle d balai polis la klu berani, sa tanya ko, ko da IC ka?

Her : *diam*.. hei... aku da IC ba.

Me: Ya, crita d balai la.

and the rest? she just cant stop talking. 1 sa cakap, 10 dia cakap, funny thing, it cannot be understand. bukan mau busybody but she's my friend, she cried in front of me, so i should do something kan.. we already report to the police station, and see how it'll going la, hehe yes, it is normal for a wife to be jelous with her husband, i do have the same feeling. BUT kalau yang t'over sampai memalukan, it is not normal kan.. kesian my friend.

31.08.09 Baby

i was truly SHOCKED to death last night! seriously!

Nasib teda sakit jantung. But im happy instead. It was weird to know your bestfriend giving a birth to a baby boy/girl without knowing she's pregnant! BESTFRIEND should know better. Im pretty sure she dont have any clue about the delivery also! or else, she would let us know. But i dunno.. The mother of two children, oh, now three children is hospitalized now. I tried to call her and the husband millions times, but failed.

Im so worried. I hope the baby and mother are fine.

Dine in Nagasaki, Japanese Restaurant.

Situated in the heart of Kota Kinabalu and having Tong Hing Supermarket and HSBC Bank as its neighbors surely caught me and my Dd's attention. Forget about Sushi King or any other Japanese Restaurant, try NAGASAKI. hehe We were welcomed by the local-sabahan-Japanese-waitress and an oldies suprano japanese song.. Both dd and me ordered 2 set of Dinners and the UNAGI - 6 slices for RM28. personally, i think it's quite expensive :P but my dd ordered it for me to try *wink* it was my first time makan unagi ni, hehe

so, here's the delicious Unagi. It tasted like a fish! the sauce was superb.

but, i couldnt eat all of it.
why?
because i keep thinking the way the unagi - ORIGINALLY looks like.

and how i imagine it still moving into my mouth! hehehehe
*urgh*
i think my dad will loves this food, just like the way Dd loves it.

Im glad i did finished everything!! hahaha
the dessert was yumie, the Japanese tea tasted like chinese tea - i think they used the same flavour? and the hospitality was cool. Im looking foward to dine in again to Japanese Restaurant, perhaps again on our next monthsary's day?? hehe

Went out from the office around 5.30pm yesterday, i was walking to Dd's car when there's this woman around 40's, walking towards me, looking straight at me..her face shows that she trying to recall if she's ever knew me.

i miss her anyhow.


the thing i will never forget about her is : she punished the whole class cruelly : wakakaka! she did actually. It was the GUYS' fault! They replaced the teacher's chair with the broken chair, hoping to see the teacher will fall down if she sits on it, we - the so-innocent GIRLS, basically dont have any idea about their plan! Few minutes later, the teacher went in, greeted the class and when she was about to sit on the chair, her bag accidentally hit the chair, and BOOM! the chair falling on the floor! She looked at us, disbelief and started yelling at us!

She brought the matter to the discipline office and punished all the guys in my class - ordered them to open their uniforms [ except the pants ] and bring their chairs to the netball's field, which happen to be situated right in the centre of all the classes! So, with their naked upper body, they standing on the chair and trying hard to cover their faces. hahaha! The funny thing was, my two girlfriends were also in the same line standing with the guys. One of my classmate report to Cikgu how these two girls being mean to her and that's why, they should be punished too! Pelapor ni haha The rest of us, stay inside the classroom and CRYING! wakakaka! Funny how it seem! The Cikgu threatened to call our parents and " Cikgu kasi black list nama kamu semua! " hehe..

Complicated.

a friend of mine told me that she dont care about his man anymore, sayin she's fallin in love, again, with her ex. i was puzzling myself whether she's being serious or she's just acting around, trying to catch her husband's attention. Or is she taking her ex as an excuse? Is it true? that the love you shared with your other half for years, can be simply, gone just like that?

she started to regret everything! It cant be happening. Can someone wash away her love to a man she had devoted for years and started to love somebody else at the same time? I cant asnwer, cause it's happen, for some people. kan? harap2 she'll do the right thing! Im not judging, but when things like this happen to a person im close with, it'll become my problem too...

The ugly truth!

Woke up soo late today! 9.27am, running on my heels to the lift, hoping i could reach the office fastest. But my stomach craving for a food, and i have to stop at 6th floor for breakfast! Reached office 10 minutes before 10am. My boss staring at me, asking ' what happen to you this morning' .. i goes ' overslept ' and she just shaking her head. I should've answer better!

Last night, Didie's mum fetched me at home around 7pm, for BBQ at his home, and went back around 12.45 am. we had a Bailey's session, i cant remember how many glasses i finished. probably 3-4? Few ya. Bailey's Original, Caramel & Mint Chocolate - and the nice one, still original. It taste goods with more ice :P i cant differentiate it with Teh C Ping at the first place, i really thought it was just a tea. but then again, it helps me goes to sleep - very nyenyak!! :)

Oh, i havent give the letter yet.

Probably should be today. or tomorrow. Courage, where are you!

Decision and Toasting :)

Back to work!

Oh, fyi.. i decided to quit my job this month! pretty sad cause i started to like this job, but i have to move on to a new direction and starts filling my times with the job im supposed to work with. Complicated yet describable. Weird to say, i feel sorry for the bos.. she seems to be so warm-hearted recently, and i dont wanna break her heart with my letter. cheh! mcm dia sedih haha.. she surely will be mad at me. Since she asked me to work here in this tourism company, at least 2 years.

Hopefully, i have the courage to send the letter by this evening! * crossed finger *

Anyway, went to Magellan Ballroom @ Sutera Habour last Saturday for both Rexie & Josephine's wedding reception. It was lovely, they both looks absolutely gorgeous and Im glad i was there to watch the memorable moment :)

MC again! i hate GASTRICK everytime it attacked me to death! Ever since i was a little girl, gastrick has been a frequent diasease for me, i should have been immune with it but i will still crying everytime because of it! huhuhuhu

Extra careful!

I was sick! fever, sore throat and flu.. i cant remember how many people trying to jokingly said i was infected by the H1N1 virus.. I really WANT 2 PULL OUT THEIR TONQUE! but my sannity acted before i lost my mind! lol. They shouldnt make a joke about this virus, what if they get infected? people died because of it, right.?. am i being too sensitive? i dunno. i just dont like people making fun of something should not be joked! oh, now i feel too naive!

well, guys.. the virus can be everywhere.. SA SIHAT SUDA AAA.. hehehe :D anyway, i have this fowarded e-mail that i wanna share with you all. WARNING: IT MIGHT BE TOO LONG! but benefits for us to know :)

A(H1N1) flu: Updates on 10 FAQs — Dr David KL Quek

1) Can we distinguish between regular and H1N1 flu, without a lab test?

No, the flu is the flu, but there are variations in presentation. Some symptoms such as cough, runny nose, fever, body aches, fatigue, vomiting, diarrhoea occur more or less in every flu patient, but may present differently by different people. Some infected people have very mild symptoms, some in between, and a small minority, probably less than 10 per cent, have severe features including the dangerous pneumonie

However, from sentinel testing and surveillance by the Ministry of Health the last few weeks have shown that almost 95 per cent of all flu-like illness are now caused by the H1N1 virus. Earlier some months ago, seasonal flu variants caused by the B and other A virus were the main causes, the bug causing most flu these few days is the A(H1N1). This appears to be the case also in neighbouring countries, meaning that the new virus is causing more havoc and symptomatic illness than previous types of flu (which are still in the community).

Because almost every flu-like illness (influenza-like illness or ILI ) is due to H1N1, the MOH is now recommending that no testing to confirm this H1N1 will now be offered.
Treat as if this is H1N1 for ILI — symptom relief for mild symptoms (paracetamol, hydration, cough medicines, etc) and self-quarantine, social distancing, be alert for complications.

Most (70 per cent) do not need any anti-viral medications such as Tamiflu or Relenza. Only severe cases need to be referred to hospital for further treatment.

2) How should doctors decide if a person be given further specific treatment for H1N1?
If after 2-3 days, fever and cough symptoms do not improve, a recheck with the doctor is recommended, especially if there are features of difficulty breathing, severe weakness and giddiness, or, if the following risk factors are present:
1. obesity (fatter patients seem to have poorer outcome and more complications)
2. those with underlying diabetes, heart disease
3. those with asthma, or chronic lung disease
4. pregnant women
5. those with reduced immunity, cancer patients, etc
6. those with obvious pneumonia features

3) Many anxious people with flu-like symptoms want to be tested or treated for suspected H1N1, but are kept waiting or sent home, without being tested. Is this practice right?


There is no right or wrong practice as this outbreak is extensive and is stretching our resources to the limit. This is also the case not just here in Malaysia , but also elsewhere around the entire world!
The recommendation is now not to spend too much time and effort trying to get tested at designated hospitals or clinics — there is probably no need to do so. I have been informed that as many as 1,000 patients queue anxiously at Sungai Buloh Hospital for testing, due to fear of the H1N1 flu.
So the message must be made clear: Most flu illness do not require confirmatory testing, and are mild and self-limiting. More than 90 per cent will get better on their own, with symptomatic treatment — just watch out for possible complications, and risk factors as mentioned above.
Our resources are limited especially for testing. This is not just for Malaysia , but globally as well. The global demand for test kits and reagents for the H1N1 (PCR) is overextended and are rationed due to this extreme demand.
Some 200 million test kits have been deployed worldwide, but this supply is critically short because of excessive demand, so most countries have to ration testing to confirm only the worst cases, so as to monitor the pandemic better.

4) Are doctors confused as to what to do in this outbreak, especially when they do not have ready access to confirmatory lab tests?


Not really. Earlier on there was some confusion as to what to do next and who to test or who to refer for further testing and admission. Now the rules are clearer.
There is no need to do any testing to confirm the H1N1 virus for any ILI — just assume that this is the case in the majority of cases. Treat symptomatically when symptoms are mild, reassure the patients and ensure that these infected patients practice good personal hygiene, impose self-quarantine and social distancing, wear masks if their coughing or sneezing become troublesome, and keep a watchful eye on whether the infection is getting better or worse.
If there is difficulty breathing and gross weakness, then patients should quickly present themselves for admission. Understandably this phase of worsening is not always clear or easily understood by everyone... But there is not much more that we can do — otherwise we will be admitting too many patients and this will totally overwhelm our health services.
But prudent caution would help to determine which seriously ill patients need more attention and more intensive care. Unfortunately however, there will be that odd patient who will progress unusually quickly and collapse even before anything can be planned — hopefully these will be few and far between.
A more important note is that all doctors and nursing personnel should be very aware that they too have to take precautions, and employ barrier contact practices, if there are patients with cough and cold during this period of H1N1 outbreak, which is expected to last a year or two. Carelessness can result in the physician or nurse or nurse-aide becoming infected!

5) Are there sufficient guidelines from the Ministry of Health to address this situation?

I think there are sufficient guidelines from the MOH. Although some politicians have blamed the MOH and the minister for being inept at handling this pandemic — in truth this is not the case.
It is useful to remember that this is an entirely new or novel virus, which no one previously had encountered before — thus its infectivity and contagiousness is quite high and almost no one is immune to this virus.
Perhaps, there will come a time when all the resources from both public and private sectors can be put to more efficient use. Some logistic problems will invariably occur, because human beings differ in their capacity to understand or follow directives, whatever the source or authority.
Also patient demands have been extraordinarily high and at times very difficult to meet — every patient necessarily feels that his flu is potentially the worst possible type and therefore requires the most stringent measures and testing...
Doctors are also unsure as to the seriousness or severity of this new ailment — and we are only now beginning to understand this better — so our less than reassuring style when encountering this new H1N1 flu is sometimes detected by an equally anxious patient and/or their relatives.
But there is only so much that we can do under such a pressure cooker of an outbreak which is spreading like wildfire! But nevertheless we should not panic, and remember that most (more than 90 per cent) of infected people will recover with very little after-effects. Possibly only one in 10 patients develop more serious problems which necessitate hospitalisation.

6) Is limiting H1N1 testing only to those who have been admitted to hospital justifiable?

I have explained the worldwide shortage of such testing kits and reagents. Also it is near impossible to test everyone, the world over. Besides, knowing now that almost all the flu-like illness in the country is due to H1N1 makes it a moot point to want to test for this, especially when most are mild.
The rationale for testing only those who need hospitalisation is to ensure that we are dealing with the true virus, and also help to isolate possible changes or mutations to this viral strain. The MOH is also constantly doing sentinel surveillance (random spot-testing at various sites around the country to determine more accurately the various virus types and spread that are causing ILI ).

7) Are we short of anti-virul drugs (Tamiflu, Relenza)? Should I take Tamiflu?

These antiviral drugs were available to most doctors during the earlier scare of the bird flu virus, but now are severely restricted, although some orders are still entertained from individual doctors, clinics or hospitals. Remember that these have been block-booked by more than 167 countries which have been shown to have been penetrated by the H1N1 flu bug.
Our MOH has actually stockpiled some two million doses of the Tamiflu or its generic form. In the last inter-ministerial pandemic influenza task force meeting, this stockpile will be bumped up to 5.5 million doses to cover some possible 20 per cent of the population.
Right now there is no shortage in the country. It is just that it is not readily available on demand for anyone just yet. The MOH is still of the opinion that this antiviral drug be used prudently and would like to register every patient given this drug.
The private sector on the other hand would like to have a looser control over the use of this drug — but we acknowledge that we should be meticulously prudent in its use. There is a genuine fear that resistant strains to this drug may develop with indiscriminate and unnecessary use — then we will all be in trouble with a drug-resistant H1N1 virus run amok!
Drug-resistant strains have been detected in Mexico , border-towns in the US , Vietnam , Britain , Australia even. So we have to be vigilant and closely monitor the situation. Right now, the very limited usage of Tamiflu gives us good reason to be optimistic.
However, because of some unusual patterns of seemingly well people dying or having very critical infections, some people and doctors are wondering if these new strains have already reached our shores... or have we been too late in instituting proper treatment...?
The rising number of deaths to 14 now is quite worrisome, but our health authorities are watching this development very closely and are also checking the virus strain to see if this has mutated. We can only hope that this is not the case, for now.

8) What are some of the problems faced by doctors in dealing with the H1N1 problem?

It would be good if every medical practitioner keeps a close tab on the H1N1 pandemic, and remain fully aware of the developments and changes, which are evolving daily. Every doctor has to be learning on the trot, so to speak, to keep up with the progress of this outbreak and its management, so that we can serve our patients better.
Logging in to the Internet regularly for more updated information will certainly help, instead of lamenting that not enough is being disseminated via the media thus far... Every doctor has to be more proactive and practice more responsible and cautious medicine during this trying period which is expected to run into at least one to two years. Importantly, look out for lung complications, and the above stated higher risk profiles, and refer these patients quickly for further care.
Easier access to antiviral drugs and their responsible use and monitoring would help allay public fears of delay in treatment, but this should be tempered with care and not over-exuberance to dish out to one and all, the precious antiviral drug, just for prevention — this may be a very bad move which can inadvertently create a worse outcome of drug-resistant bugs.
However, in the light of the very quick deterioration of some young patients who have died, it might be prudent to use antiviral treatment earlier and more aggressively.
We look forward to the specific H1N1 vaccine, when it does come our way, probably towards the end of the year. In the meantime, encouraging those in the front-line, heart or lung patients and frequent travellers to have the seasonal flu vaccination is a useful adjunct to help stem the usual problems from other flu types.

9) Are we doing everything that should or needs to be done?

Yes, if you check what other nations are doing, we are doing relatively well. We are not overstating the dangers and we have been quite transparent on the possibilities of this pandemic. Earlier, many agencies and even the public and doctors have accused us of exaggerating the pandemic, and our response was dismissed as being too much, even over the top! Unfortunately, it was only when some deaths occur that many are now decrying that we have done too little!
Also if you are quite honest about it, just compare with the countries globally, and you will notice that no one health or government authority has got this right, spot on.
We are all learning about this novel flu pandemic, and each country's response is coloured by its past experiences. In Hong Kong , China , Vietnam , Singapore and Malaysia we have had the SARS outbreak, so we are necessarily more paranoid! Also here the experience is that flu does not usually cause death in our community, unlike the west where seasonal flu kills some hundreds of thousands every year!
So the fear factor for this H1N1 flu is not nearly as great in the West, although it is slowly sinking in that its contagiousness and infectivity is far greater, and fears of its reassortment to a more virulent mutant form are growing, into the so-called second and/or third wave of this pandemic, but we will not know until a year or so down the line.

10) Is the public in general doing enough to help in controlling the outbreak?

I think the public is now reasonably well-informed as to this H1N1 pandemic. Perhaps, they are too well-informed, that they have a fearful approach to this virus. But the proper thing is not too over-react and to panic, although I know this does sound easier said than done.
It is almost a certainty that this flu will spread within the community — in schools, universities, academies, factories, work places, offices, etc. WHO has projected that possibly some 20-30 per cent of the population worldwide will become infected by this novel flu bug, after studying various models of spread of past infections — the huge and very rapid spread worldwide is mainly due to air travel. While older flu pandemics took six months to extend to so many countries, this H1N1 flu did so in less than six weeks!
In the worst-case scenarios of course, this outbreak will be alarming — hospitalisations may be required for 100,000 up to 500,000 Malaysians, with perhaps as many as 5,000 to 27,000 infected patients (depending on the case fatality rate or either 0.1 to 0.5 per cent) succumbing to this illness.
But because we have been monitoring closely and containing the outbreak thus far, with heightened awareness and greater social responsibility, it is possible to ameliorate the infectivity, spread and fatality that will unfortunately accompany this pandemic... Just how successful we will be in limiting these adverse outcomes remains to be seen, but we can be hopeful.
How can the public help? First learn and acquire good personal hygiene. If sick, please be responsible and stay at home, even in your own room where possible, wear a face mask (a cheap three-ply surgical mask will do, because large droplet spread is the main danger). Do not go out, practice what is now known as social distancing (about three metres from anyone), and be socially responsible, don't go to public places and infect others — for young people this would be hard, but absolutely necessary — the spread is most rampant in this age group between 16 and 25 years.
When the illness does not go away after a few days or when you are deteriorating, get to the nearest hospital. Most importantly, be very aware and responsible!
Finally, keep abreast of all new developments, because these are evolving all the time. With keen awareness, prudent care, early detection and social responsibility, correct and prompt use of antiviral and other support medical care, and later mass specific vaccination, we can overcome this novel H1N1 flu! But it will take time, patience, public cooperation, much concerted effort and consume great resources.
